PHYSICALS are required before attending camp. If you did not have a physical on file, you are required to do so in order to participate in marching band or guard.
· You will also be required to complete the Pre-Participation Forms online including: Concussion and Sudden Cardiac Arrest Acknowledgement, Informed Consent, Pledge and Consent, Permission to Photograph/Videotape, Disclaimer for Risk Injury, Health Insurance and Athletic Handbook.
· To be eligible to participate on the first official day of practice (this includes going to camp) please be sure that:
1. You have turned in your completed physical to the MHS athletic office.
2. You have completed the online paperwork (All 5 forms)
